Personal Information will include your Name, Email Address and dietary eating habits as documented by you. I am a qualified Level 3 & 4 Nutritionist, this enables me to offer you nutrition advise based upon the current government guidelines, it is my responsibilty to ensure that advice I provide you with is up to date. I can advise and recommend you with recipes. I am not a Dietician so I can not provide you with a diet plan and I am unable to advise upon special dietary requirements. If more help is required regarding this it is recommended to contact your doctor or Registered Dietician for further advice. Personal Information included in the Dietary Assessment Form will not be replicated or used in any other way. Personal Information will be stored electronically and securely by password. Personal Information collected is only used for identity purposes and to establish your current feelings and mood, as interpreted and documented by you. This information will be handled respectfully and sensitively. From the information provided from The Mindfulness Questionnaire I can establish your current self-identified mood and feelings, is used as a marker to build upon a more improved mood and feelings as the programme progresses, allowing for noticeable progression. I am not a Mental Health Professional nor am I qualified to diagnose or advise on Mental illnesses. I am, however, able to look to improve your mental health through exercise, educate you with its benefits to improving your mental health and give you my own impartial views and from my experiences. Information received will be collected without judgement or opinion. Personal Information included in the Mindfulness Questionnaire will not be replicated or used in any other way. Information collected will not be shared with any other third parties without your written consent in advance and will be kept confidential.

Personal Information will be shared by communications with myself (Jami) and you. This will be a live video, provided through the video networking app "Zoom". This will not be recorded or replicated. I may, however, make some notes from information shared following questions and conversations exchanged. This information will only be relevant to the constructing of the Exercise Plan, which will include your current activity levels, your preferences, goals, and needs. This will help to establish the best suited Exercise Plan for you. These notes will be stored by lock and key.
